Participant Waiver, Release of Liability, and Assumption of Risk Agreement

1. Assumption of Risk

I, the undersigned, understand and acknowledge that participation in any physical training activities, including but not limited to boxing, martial arts, MMA, strength training, and general gym use at Fighting Chance Training Center, carries with it inherent risks of injury, illness, disability, or death. This includes, but is not limited to, cuts, bruises, sprains, fractures, head injuries, cardiac events, or other forms of trauma.

I knowingly and voluntarily assume all such risks, known and unknown, associated with my participation.

2. Release of Liability

I hereby release, waive, discharge, and covenant not to sue Fighting Chance Training Center, its owners, officers, employees, coaches, volunteers, sublet partners, and independent instructors (collectively, the “Released Parties”) from any and all liability, claims, demands, causes of action, or damages for any injury, loss, or damage to person or property, including death, that may arise from my use of the facility, equipment, or participation in any activities, whether supervised or unsupervised, regardless of cause, including negligence.

This release extends to all activities inside or outside the facility, during classes, open gym, private training, events, or otherwise.

3. Sublet & Independent Coaches

I acknowledge that some classes and services may be provided by independent coaches or organizations who lease space at Fighting Chance Training Center. I agree that these individuals or entities are also not liable for any injuries or damages I may sustain during participation in their offerings.

4. Health Status

I affirm that I am in good health, physically fit to participate, and free from any medical condition that would increase my risk of injury. I will inform staff or instructors of any relevant health concerns before participation.

5. Minor Participants

If signing for a minor, I affirm that I am their legal guardian and agree to the terms on their behalf. I accept full responsibility for the minor’s safety and behavior at all times on the premises.

6. Indemnification

I agree to indemnify and hold harmless the Released Parties from any loss, liability, damage, or cost they may incur arising out of my participation, or that of any minor for whom I am responsible.
