Participant Waiver, Release of Liability, and Assumption of Risk Agreement

1. Assumption of Risk
   I, the undersigned, understand and acknowledge that participation in physical training activities—including but not limited to boxing, martial arts, MMA, strength training, and general gym use—carries inherent risks of injury, illness, disability, or death. These risks may include cuts, bruises, sprains, fractures, head injuries, cardiac events, or other forms of trauma.
   I knowingly and voluntarily assume all such risks, both known and unknown, associated with my participation.

2. Release of Liability
   I hereby release, waive, discharge, and covenant not to sue Fighting Chance Training Center, its owners, officers, employees, coaches, volunteers, sublet partners, and independent instructors (collectively, the “Released Parties”) from any and all liability, claims, demands, causes of action, or damages arising from any injury, loss, or damage to person or property, including death, that may occur from my participation in any activities, whether supervised or unsupervised, and regardless of cause, including negligence.
   This release applies to all activities inside or outside the facility, including classes, open gym, private training, events, or any related activities.

3. Sublet & Independent Coaches
   I acknowledge that some classes and services are provided by independent coaches or organizations who lease space at Fighting Chance Training Center. I agree that these individuals and entities are also not liable for any injuries or damages I may sustain during participation in their activities.

4. Health Status
   I affirm that I am in good health, physically capable of participating, and free from medical conditions that could increase my risk of injury. I agree to inform staff or instructors of any relevant health concerns prior to participating.

5. Minor Participants
   If signing on behalf of a minor, I affirm that I am the minor’s legal guardian and agree to all terms of this waiver on their behalf. I accept full responsibility for the minor’s safety, conduct, and compliance while on the premises.

6. Indemnification
   I agree to indemnify and hold harmless the Released Parties from any loss, liability, damage, or cost incurred due to my participation or that of any minor for whom I am responsible.
